2018 - 2019 Clinical Educator of the Year Award
Candidates may nominate themselves or be nominated by a peer (OR team member, school administrator, etc.) Either the candidate, or the practitioner who nominated the candidate, is responsible for providing complete documentation (listed below). Incomplete packets will not be considered. The submission deadline is December 1, 2018. Applications may be mailed or emailed to email@example.com.
Candidate must be a full- or part-time clinical or lab instructor at an accredited school. (Students must be eligible to sit for the NBSTSA surgical technology certifying examination).
Self-nominated candidates must write a 500-word essay explaining why he/she merits this distinction and provide one letter of recommendation from a peer (educator, school administrator, not necessarily from your institution). Email essay to firstname.lastname@example.org.
Peer-nominated candidates must submit two letters of recommendation from a peer (OR team member, school administrator, not necessarily from your institution) with the application. This should also include any involvement with AST. Letters should be emailed to email@example.com.
- Current curriculum vitae
- State assembly involvement (describe offices held, attendance at meetings, presentations given), etc. (This is important feedback since we encourage students to become involved early in their state assemblies).

In addition, nominees are encouraged to solicit a maximum of two student comments which can be forwarded to the Foundation confidentially to email@example.com.
